Please Note: Currently the Department of Veteran Affairs does not offer an online VA Certificate request. Your signature is required so the only way to get it is to follow the instructions below.

1. Obtain the VA Eligibility Request form. (Click to download)
2. Complete and send to:

Winston-Salem, NC 27120

Note: You will also need to send a "Proof of Service" or a DD Form 214 along with your request. The DD-214 is issued upon retirement from the military. If you are still on active duty you can get a Proof Of Service letter from your servicing personnel office.

3. You should receive your certificate in about 10 days.
